2020-2021 OMHA Player Experience Survey

The Ontario Minor Hockey Association is continually striving to improve our programs for our membership. We value your thoughts and opinions on your participation in the OMHA from this year.
The objective of this survey is to obtain information from participants to gain a better understanding of your experience this season.The survey will take less than 10 minutes to complete.
All information provided will be anonymous and remain confidential within the structure of OMHA programming. We appreciate you taking the time to respond.
