Attention Parents!!
If your child is wishing to try out for a AAA team this season:
1) please fill out the attached Permission to Skate form online
2) send the completed form as a fillable document to the OMHA rep Karl Cousineau email:
[email protected].
Note is has to be sent as a fillable document so Karl can sign in online and send it back to you.
3) you still need to register with Sturgeon Lake Minor Hockey. Registration will be opening soon.
